1.   Schedule an interview appointment on this website and print your appointment confirmation page. For a

2.   Fill out the DS-160 Form at https://ceac.state.gov/genniv/.The new DS-160 Form replaces forms DS-156,
      DS-157, and DS-158.

3.   After successfully filling out the DS-160 Form, you must print out the DS-160 confirmation page. For a
      sample DS-160 confirmation page, please click here.

• On the DS-160 Form, you will be prompted to select a location where you will be submitting your application Your city selection on the Form must be the same as your interview location PLEASE NOTE: Locations are listed in alphabetical order by city name: Brasilia, Recife, Rio de Janeiro and Sao Paulo.


• You must fill-out the DS-160 Form on a computer that is connected to a printer.


• If you make changes after filling out the form, you will need to print a new confirmation page and bring to your interview.


• Your DS-160 Form must be completed no less than 2 days prior to your appointment date.


• Applicants who do not present the DS-160 confirmation page on their interview date will NOT be interviewed and MUST reschedule their appointments for the next available date.


• YOU MUST BRING BOTH CONFIRMATION PAGES FOR EACH APPLICANT TO THE VISA INTERVIEW


• Please provide accurate and complete responses to all questions on the DS-160 Form, especially in fields such as "Name and Surname", "Date of Birth", "Sex", "Other Names and "Visa Class." You will be given an opportunity to review the information before submitting the form electronically. ANY errors on the form (even minor typographical errors) or incomplete information may result in a rescheduling of your appointment. You will NOT be allowed to interview until you have corrected all fields.
