The Nonimmigrant Visa Application Form DS-160 must be completed electronically. The form is available at the following Website address:

https://ceac.state.gov/genniv/

After completing the Form DS-160 you must print the Confirmation Page that contains a barcode and bring it with you to your scheduled appointment.
