DS-156 - Nonimmigrant Visa Application Form
The DS-156 form is to be filled out by all nonimmigrant visa applicants.

Note: if you scheduled your interview through the system, you have already filled out the DS156 form, therefore you just need to log in to the system, update your registration and print out your form.

DS-157 - Supplemental Nonimmigrant Visa Application Form
The DS-157 form is to be filled by all nonimmigrant visa applicants 16 years-of-age and older, regardless of nationality or where they apply.
DS-158 - Contact Information and Work History for Nonimmigrant Visa Applicant
The DS-158 form is to be filled by all student and exchange visitor visa applicants, regardless of nationality or where they apply.
